After a workplace accident in New York, you do not have to be treated in New York. Dr. Dariusz Nasiek’s practice, authorized to treat patients under the New York State Workers’ Compensation system, sees patients at its offices in Englewood, Garfield and Elizabeth, New Jersey – in Polish, from the very first call.
The job is in New York: construction, the railroad, a warehouse, a restaurant, office cleaning. Home, family and the children’s school are in New Jersey. This split bothers no one until the day of the accident. Many people then assume that since the accident happened in New York, treatment has to be in New York too. In practice, it works differently.
Treatment closer to home
Many patients have a Workers’ Compensation case, a claim under the employer’s accident insurance, handled in New York, while being treated on the New Jersey side. Treatment after an accident is a series of visits spread over weeks: examination, results, rehabilitation, follow-up. When every visit means a drive through the tunnel and traffic, patients start skipping them – not because the pain is gone, but because they no longer have the strength to travel. Interrupted rehabilitation rarely gives the same result as treatment carried through consistently.
Talking in Polish is part of the examination
In pain medicine, the patient interview is a diagnostic tool. Is the pain burning or shooting, does it wake you at night, where does it radiate – these details show where to look for the cause, and they are the hardest to describe in a foreign language. In our offices, patients can speak Polish from the first call, through the examination, to the review of results. A patient who understands the treatment plan sticks to it.
From the first call to the first examination
The first step is a call from the patient or a referral from the attorney handling the case. The first visit is a full neurological and physical examination; we try to schedule it within the same week, because an early examination matters both for treatment and for how the injury is described in the chart. After an accident, the first consultation is free: neurological, orthopedic or for spine pain.
At the same time, we keep the medical documentation: the examination report, the mechanism of injury, an assessment of its consequences and the treatment plan, based on objective test results. Legal matters are handled by lawyers, with whom we work closely; our part is medicine.
Surgery is the last step, not the first
The most common question in the office is: “Doctor, will I need surgery?” Treatment is easiest to picture as a ladder:
- Diagnosis – examination, MRI, CT, X-ray, EMG/NCV, and for head injuries EEG and memory tests. Mild traumatic brain injuries (TBI) are the ones most often missed after an accident; in our team they are assessed by neurologist Dr. Rikki Racela, MD.
- Conservative treatment – medication, physical therapy, manual therapy and rehabilitation tailored to the injury and the type of work.
- Injections and blocks – given under imaging guidance exactly where the pain originates, including epidural injections, spinal joint blocks and PRP.
- Minimally invasive procedures – performed through the skin, without opening the spine, such as RF ablation.
Patients can stop at any step, and many do. When surgery is needed, we refer to trusted surgeons and coordinate further treatment.
When to call right away
Progressive weakness in an arm or leg, spreading numbness, problems with bladder control, and after a head injury a worsening headache, vomiting or confusion require immediate medical help – with sudden symptoms, call 911. It is also worth getting examined when pain does not ease after two or three weeks, or appears a few days after the accident, because adrenaline can mask it.
