Dr. Nasiek spoke about Traumatic Brain Injury (TBI) and its profound impact on life and life expectancy. He emphasized the importance of thorough evaluation and diagnosis following a TBI. The initial post-evaluation step involves establishing a plausible injury mechanism by asking the patient to describe the events surrounding the injury and listening carefully for details of a concussive force and its intensity.
Next, Dr. Nasiek highlighted the importance of querying signs and symptoms to determine whether the patient’s mental status was altered immediately after the impact. Examples of questions to ask include: "Do you remember the moments just after the impact?" "Were you confused or unsure about where you were and what was happening?" and "Were you able to think clearly about what to do after the accident?"
Lastly, he stressed the need to rule out confounding factors that could account for the acute alteration in mental status. Questions such as "Were you drinking alcohol or using drugs just before the accident?" and "Did you feel panicked or scared?" help differentiate between brain injury and other causes of altered mental status.
