The scholarship for undergraduate students of Polish descent in USA

Application must be postmarked on or before 10 January , 2015

The scholarship for undergraduate students of Polish descent.
All applicants must:

1. Be of a Polish descent.
2. Have own or family membership of Polonia of Long Island.
3. Students awarded Polonia of Long Island Scholarship twice, may not apply.

To apply for the scholarship, please submit the following to:

1. A completed Application.
2.Attach a typewritten, double-spaced, original essay written in English.

(One out of two to choose)

I.List Polish Noble Prize winners along with the field of study they received the award for. Choose two and describe, why they are of most interest to you.
II. The Warsaw Uprising as seen in literature, movies and music. Put your name and address information on the cover sheet of the Essay. (Do NOT put your name or any identifying information on any page of the essay.)
III. Provide an official transcript from all colleges attended (copies will not be accepted) with minimum GPA 3.2.
IV.List and describe extra curricular activities and other current interests. Use an additional sheet of paper if needed.
V. Enclose two sealed envelopes with letters of recommendation. One of them must be on an institution letterhead from faculty member attesting to your academic qualities. The other one can be from an individual of your choice who has known you for at least two years.
VI. If there are special circumstances you wish to bring to the attention of the Scholarship Committee, please explain on a separate sheet of paper.
VII. Please enclose self – addressed envelope with a stamp to receive confirmation of your application by mail, or your e-mail address.
